Home support services
What is HACC?
All Home and Community Care (HACC) services are now through Westmont Aged Care Services, who aim to keep you living at home and in your community as independently as possible.
First, they find out what is important to you and what you want to achieve.
Then together, help put in place the right goals and solutions with you.
Westmont's HACC services take an active approach to service delivery, with carers working with the individual.
The HACC service has two main components: assessment and service provision (including Meals On Wheels, personal care, respite, domestic support, Planned Activity Group (PAG), home modifications and home maintenance).
These services are aimed at providing a basic level of support, which actively encourages individuals to work with the carer to enhance their quality of life and independence.
Support services available
A closer look at support services available from Westmond Aged Care include:
Personal care
Working with you to manage personal tasks such as having a shower and getting dressed.
Domestic support
Working with the client to achieve the following tasks:
- Shopping;
- Meal preparation;
- Vacuuming and floor sweeping/mopping;
- Cleaning of bathroom, shower and toilet; and
- Bed-making and changing linen.
Home modifications
Rails, ramps and safety adjustments can be undertaken following occupational therapist recommendations.
Respite care
Allows the carer to attend social functions and be out of home for activities like shopping. Alternatively, the client can be taken out of the home for a social outing. This is dependent on the client’s level of care needs.
Meals On Wheels
There is a choice of fresh or frozen meals and therapeutic diets available, and these are delivered to your nominated address.
Home maintenance
This includes lawn mowing.
Planned Activity Group (PAG)
Clients come and enjoy a friendly, social environment in a setting with other people. Activities including carpet bowls, painting, gentle exercise and cards. A freshly prepared meal is provided. The meals are cooked on site by volunteers and/or staff.
